Since the discontinuation of Step 2 CS in 2021, ECFMG has used a “Pathways” system to verify that IMGs have the clinical and communication skills needed for US residency training. Understanding which pathway you qualify for — and starting the process early — is one of the most important things you can do for your match prospects.

The Requirements That Apply to Everyone

Regardless of which Pathway you pursue, all applicants must meet these requirements:

The 7-Year Rule

You must complete Step 1, Step 2 CK, and your Pathway requirements within 7 years of your first USMLE pass. ECFMG does not send reminders. If your Step 1 pass is approaching 7 years old and you haven’t completed everything else, you may need to retake it. Track this yourself — it’s your responsibility.

Which Pathway Is Right for You?

Pathway 1: Already Licensed to Practice

For IMGs who currently hold or recently held a full, unrestricted medical license in another country. For 2026, the license must have been valid on or after January 1, 2021. This is typically the simplest pathway — you submit documentation of your license and ECFMG verifies it. Start requesting your Certificate of Good Standing early, as some licensing authorities take weeks to process these requests.

Pathways 3, 4, and 5: Recent Graduates of Eligible Schools

For students or recent graduates (graduation date on or after January 1, 2023) of medical schools that meet specific accreditation criteria. The key requirement is that an authorized official at your medical school must attest to your clinical skills. Pathway 3 covers schools accredited by a WFME-recognized agency, Pathway 4 covers NCFMEA-comparable accreditation, and Pathway 5 covers joint degrees with LCME-accredited US schools.

Pathway 6: Mini-CEX Evaluations

For applicants who don’t qualify for other pathways, or who previously failed Step 2 CS. Pathway 6 requires licensed physicians to evaluate your clinical skills through in-person Mini-CEX encounters. Identify your physician evaluators early — they must meet specific criteria and accept your request through the ECFMG system before you can complete encounters.

OET Medicine: The Universal Requirement

Every Pathways applicant must pass OET Medicine, which tests healthcare-specific English proficiency across reading, writing, listening, and speaking. Key points:

This is where many IMGs get caught off guard. ECFMG processing times vary significantly based on documentation completeness and seasonal volume:

Strategic Timing

Aim to have your ECFMG certification complete before September 24 (when programs start reviewing ERAS applications). Many programs filter by certification status. Being certified on day one means your application gets seen immediately.

Pathway Expiration

Your accepted Pathway has an expiration date based on the season it was approved. For 2026 Pathways, certification expires December 31, 2028. Once you complete 12 months of ACGME-accredited training in the US, your certification becomes permanent.

Frequently Asked Questions

How long does ECFMG certification take once OET is passed?

If all other requirements are met (Step 1, Step 2 CK passed, Pathway documentation complete), certification can be finalized within 2–4 weeks of OET scores being received. During peak season (December–January), expect closer to 4–6 weeks.

Can I apply to ERAS while my Pathways application is pending?

Yes. Your ERAS application will show your ECFMG status as pending. Some programs will still review your application; others filter by completed certification. The earlier you get certified, the more programs will see your complete application.

What if my previous Pathway expired?

You’ll need to submit a 2026 Pathways Revalidation Application. Revalidation is only available through Pathway 1 or Pathway 6 — you cannot revalidate through Pathways 2–5. You’ll also need a new satisfactory OET score (on or after January 1, 2024).
